Manager.
Key Responsibilities
Assess, plan, implement, and coordinate case management activities
to evaluate and enhance member wellness
Conduct comprehensive assessments considering various health
conditions and social indicators
Collaborate with stakeholders to overcome barriers and ensure
effective care planning
Required Qualifications
Registered Nurse with an unrestricted nursing license and
multi-state/compact privileges
3+ years of clinical/nursing practice experience
1+ year of medical case management, care coordination, or discharge
planning experience
Proficiency in using personal computers and MS Office Suite
applications
High-speed internet access with the ability to hard wire a
computer
