Job Description:

Under the direction of and in collaboration with the Manager Medical Office, Director of Operations, and the Executive Director, the Lead Care Manager, RN supports the Transitional Medical Clinic by providing patient care within the scope of practice defined by the California Board of Registered Nursing. Responsibilities also include support as needed with the Comprehensive Care Clinic, and assisting both internal and external customers in conjunction with the development of the Transitions of Care teamsEssential Functions:Follow the Care Management Process, a collaborative model including patients, nurses, social workers, physicians, other practitioners, care givers, and the community. (70%)Meeting the patient's needs utilizing assessment, planning, implementation, coordination, monitoring and evaluation to develop an individualized care plan. .Advocate for and assist the patient in the achievement of optimal health, access to care, and appropriately utilizing resources.Medication Reconciliation and Medication Adherence EducationDisease Education and CoachingAdvance Directive/End of life discussionsReferral/Authorization ManagementSetting up Home Health, DME, testing, and medical appointmentsSupport with psychosocial needsProvide personal and chart-based case management, coordinate outside needsCoordinating Community and Health System programs and resourcesUnderstand and attempt to remove any appointment barriersCoordinating Care with PCP, Specialists, and ancillary teams across health system.At appropriate and repeated intervals, assess and reassess the patient's progress utilizing telephone, othertechnologies and resources.Depending on the progress, encourage appropriate interventions to obtain optimal outcome.Modify the plan of care as necessary with the healthcare team, family, and providers.Utilized as a resource when supervisor in unavailable. Responsible for problem solving and staffing / scheduling changes as needed. Displays leadership skills, mentoring staff to achieve success (20%)Consistently demonstrates efficient use of time and resources within established budget parameters, while actively participating in identification and implementation of cost saving strategies. (5%)Anticipates provider and/or patient needs and responds appropriately. (5%)Additional Responsibilities:Facilitates care along the continuum through effective resource management.Some Direct Patient Care for CCC as needed if CCC Nurse Practitioner is UnavailableMaintains current knowledge of SJHMG policies and proceduresAttends and participates in all mandatory meetings/in-services.Performs all other duties as assigned.Minimum Position Qualifications:Education:Requires a High school diploma or equivalent.Graduate of an Accredited School of Nursing required.Experience:Requires three (3) years of OON CM experienceLicensure/Certification:Active RN license in the State of California requiredActive CPR certificate requiredComputer Skills: Basic computer skills including Microsoft Word, Windows, and Excel required.Knowledge / Skills / Abilities:Excellent customer services skills with a genuine caring approach to helping people.Ability to maintain a positive attitude with changing conditions.Excellent organizational abilities.Excellent written and verbal communications skills to communicate effectively with individuals at all levels of the organization.Ability to work under general supervision.Must possess a professional and mature demeanor.Ability to work in a fast-paced department and handle multiple tasks, work with interruptions, and deal effectively with confidential information.Preferred Position Qualifications:Education:BA in Nursing preferred.Experience:Previous experience in a leadership role preferredLicensure/Certification:CCM Certification preferredMCG Certification preferredSt. Joseph Heritage Healthcare (SJHH) is one of California's most respected medical groups.
