We are looking for a Senior Speech/Language Pathologist for the Rehabilitation Outpatient Department at St. Jude Medical Center.
Location: Fullerton, CA
Work Schedule: Full Time - 80 Biweekly Hours
Shift: 8-hour, Days
Job Summary:
Under the direct supervision of the Program Manager and consultative supervision of the Speech Pathology Practice Manager, the Speech Pathologist II is responsible for providing quality care for patients receiving Speech Pathology services. This includes detailed evaluation, treatment planning, goal setting, treatment implementation, discharge planning to transition the patient toward maximum independence. The Speech Pathologist II documents according to program and professional standards. The Speech Pathologist II also demonstrates advanced treatment skills, and functions as a clinical instructor in the development of students and interns. The Speech Pathologist II follows an individualized performance development plan to maintain current and competent in critical areas and support program goals, hospital goals and objectives, hospital functions and philosophy.
Essential Functions:



  • Screens, accepts and responds to Referrals, makes referrals to appropriate resources and educates potential referral sources about the scope of services and the process of initiating services.


  • Performs complete Evaluations and re-assessments in a timely fashion to obtain data necessary for patients with various diagnoses on assigned program. Utilizes specialty assessments to obtain detailed data for patients on assigned program, and generalizes basic evaluation skills when floating to other programs.


  • Establishes Intervention Plan and Goals that are clear, measurable, behavioral, functional, contextually relevant, and appropriate to the client's needs, desires, and expected outcomes. Incorporates patient and family goals.


  • Implements treatment Intervention through specified purposeful activities and therapeutic methods that are meaningful to the patient. Modifies the process to reflect changes in client status, desires, and response to intervention.


  • Completes all required Documentation according to Program and Professional standards, reflecting patient-centered care by relating interventions to established goals and justifying skilled need.


  • Performs Discharge Planning to facilitate smooth transition to next level of care; assures patient status is maintained or enhanced through home program instruction, caregiver training, and referral to appropriate follow-up resources.


  • Meets program Productivity Standard, outcome expectations, and completes a Program specific project. Participates in data collection and analysis.


  • Directs, Monitors, and Supervises personnel and students/interns and co-signs notes as indicated.

  • Participates in Development of others (i.e. student/intern supervision, staff orientation and development via 1:1, in-services, and/or competency assessments)

    Skills:


    • Able to perform at established guidelines for program specific requirements.


    • Preferred: Multilingual.


    • Preferred: Knowledge of TJC, CARF, and Title XXII standards.

    • Preferred: Competence in computers and computer applications (word-processing)

      Minimum Position Requirements:
      Education: Master's Degree - Graduate of an accredited program in Communicative Disorders or Comparable education (ASHA approved).
      Experience:


      • 3 years Clinical experience working as a Speech Pathologist.


      • Experience completing a project or special assignment related to program or staff development.


      • Experience serving as a clinical instructor for interning students and participated in presenting in-services to staff/students.

      • Participated in at least one community service event over the past year.

        Licenses/Certifications:


        • Certificate of Clinical Competence from the American Speech Language Hearing Association.


        • Current and valid license as a Speech Pathologist, issued by the State of California.

        • Certification of completion of training in Cardiopulmonary Resuscitation (Class C)-BLS
