As a Registered Nurse Case Manager (RNCM), you will support patients with chronic and complex conditions, lead a dedicated Care Management Team, and help strengthen healthcare access within a Tribal community known for its resilience, culture, and deep sense of connection. What You’ll DoLead & Inspire - Oversee the Care Management Team (Patient Advocates Care Coordinator) - Lead hiring, training, performance evaluation, and goal setting - Maintain team productivity, schedule coverage, and departmental workflow - Participate in continuous quality improvement and policy development Deliver Exceptional Patient Care - Complete comprehensive assessments and create individualized care plans - Triage referrals promptly and coordinate urgent and ongoing services - Build strong relationships with patients, families, and multidisciplinary teams - Use NextGen EHR for documentation, tracking progress, and care coordination - Connect patients to community resources, financial assistance, and support programs - Provide RN support at both clinics during staffing shortages ??
