Job Description:
A company is looking for a Telephonic Care Navigator.
Key Responsibilities
Completes telephonic outreach to DSNP/CSNP members and assists with
care gaps
Educates members on healthcare access and links them to community
resources
Maintains schedule adherence while providing excellent customer
service
Required Qualifications
High School Diploma / GED or equivalent work experience
Must be 18 years of age or older
1+ years of call center or telephonic customer service
experience
1+ years of professional experience using telephone and computer in
an office setting
1+ years of experience in healthcare, insurance, social work, or
community outreach
