Job Description:
A company is looking for a MDS Nurse Care Manager - Post Acute
Care (CST or MST).
Key Responsibilities
Manage overall care management of patient caseload, ensuring
efficient skilled nursing stays
Facilitate transitions for nephrology patients from Skilled Nursing
Facilities to home or community
Collaborate with healthcare providers and staff to address patient
needs and monitor progress
Required Qualifications
Associate degree in nursing required; bachelor's degree
preferred
Current RN License required; Compact License preferred
Minimum 3-5 years of experience in telephonic care management and
skilled nursing facility care
Experience in kidney care and RAC-CT certification preferred
Strong understanding of skilled nursing facility operations and
regulations preferred
